A runner covers ten kilometers and calls it a session. A nurse covers something similar between the start of a shift and the end of it, on hard hospital floors, and calls it Tuesday.

Work is the most repeated physical activity in your life. It happens five days a week, most weeks of the year, for decades. Nobody warms up for it, nobody tapers, and almost nobody thinks about the equipment. Yet the load it puts on your feet dwarfs anything most people do at the weekend, because the defining factor here is not intensity. It is duration, repeated without a season ending.

Standing looks passive, and it is anything but. Holding your body upright is continuous muscular work, and doing it on concrete, tile or hospital flooring means the surface gives back every bit of force you put into it, because nothing about a hard floor absorbs anything.

Walking on that surface changes the equation only slightly. What changes it more is the repetition. Thousands of steps a day, in the same shoes, on the same ground, with the same misalignment if you happen to have one. A foot that rolls slightly too far inward will do so every single time, and the correction is made a little further up the chain, in the knee, the hip and eventually the lower back.

Then there is the footwear. Safety shoes are engineered to protect you from what is outside the shoe, and they do that job well, with reinforced toe caps, puncture-resistant soles and rigid construction. Very little of that engineering is aimed at the foot inside. Dress shoes are worse still, built to a silhouette rather than to a foot. In both cases, the part touching your sole is generic, thin and interchangeable.

The result is familiar to anyone who works on their feet. Burning soles by mid-afternoon, heavy legs on the way home, and that first painful step in the morning that you have started treating as normal.



Typical problems that can develop 

Foot and ankle 

•     The foot rolls too far inward or outward across thousands of daily steps, often caused by flat, low, or splayed feet (overpronation / supination)

•     The connective tissue along the sole becomes inflamed through hours of continuous load, which can lead to a bony growth on the heel bone over time (plantar fasciitis / heel spur)

•     Pressure concentrates on individual points instead of spreading across the sole, causing burning and forefoot pain (metatarsalgia)

•     Feet swell over the course of a shift and shoes that fit in the morning press by the afternoon

•     The tendon at the back of the lower leg is irritated by long hours in flat, unsupportive footwear (Achilles tendinopathy)

•    Insufficient foot support transfers directly to the knee joint (malalignment)

•     Hard floors return force that has to be absorbed somewhere, and the knee is next in line

•     The kneecap is pulled off its intended track by inward rotation of the lower leg (patellofemoral pain)

•     Chronic misalignment increases the long-term risk of joint wear (osteoarthritis)

•     When the foot and knee compensate, the hip works asymmetrically and overloads one side (leg axis misalignment)

•     Standing posture shifts as fatigue sets in, and the lower back takes the difference

•     Work is rarely symmetrical, and the side you favor, lift with or stand on builds imbalance over years (asymmetric overload)

•     Muscles develop unevenly, leading to tension and pain (muscular imbalances)
